- The patient is responsible for his/her own behavior and is expected to treat the staff and other patients with respect and courtesy.
- The patient is responsible for making, and promptly keeping and calling to cancel or change appointments. The patient is also responsible for informing the staff of any changes in their address or phone number.
- The patient is responsible for releasing all information related to past illness, treatments, and medications to assist the staff in the provision of better health care.
- The patient is responsible for following the directions, prescriptions, and recommendations given by the doctors, assistants and other health center professionals.
- The patient is responsible for informing the clinic of their health insurance and/or provide a medical coupon.
- The patient is responsible for providing supervision of their children.
- The patient is responsible for knowing the clinic rules and regulations, i.e., hours of operation, eligibility rules, etc.
- The patient is responsible to know what afterhours care is available and how to obtain it.
- The patient is responsible for obtaining any referral information, i.e., appointment time and date and for keeping appointments or calling to cancel or change appointments.
