Eligibility:
To be eligible for direct care services you must be enrolled in a federally recognized tribe or provide proof of Indian descent from a Federally recognized tribe or descendent of 1st or 2nd degree. Non-Tribal are only eligible when pregnant with an eligible patient during pregnancy and weeks after birth. And when controlling a public health threat. A non-Native significant other and/or household member may also be eligible for treating substance use disorder and assistance with health benefits sign-up.
Required Documents:
Patients are responsible for providing accurate registration documentation including the following when requested:
- Patient Registration form completed
- Tribal Enrollment
- Birth Certificate (if under 18 years old)
- Proof of Residency
- Insurance Coverage
Every patient who receives services through the LTHC must check-in with patient registration at each visit to update patient accounts and provide insurance card.
