Clinical Social Workers

Lummi Tribal Health Center’s Clinical Social Work team is here to support you.  We work closely with LTHC Primary Care Providers, Psychiatry and other LTHC staff.  Clinical Social Workers receive referrals from Primary Care Provider’s to provide brief therapy for a variety of conditions or situations including:

  • Insomnia
  • Parenting challenges
  • Grief and loss
  • Anxiety

We also assist in connecting you with longer term services providers such as Lummi Behavioral Health, Lummi Counseling Services or other therapy providers.

Additionally, Clinical Social Workers can meet with you to assess needs and assist in coordinating services and helping you to develop a plan to meet your health and wellness goals.  Talk to your Primary Care Provider (PCP) to discuss a possible referral.

Care Navigators

Lummi Tribal Health Center’s Care Navigators work closely with LTHC Primary Care Providers, Clinical Social Workers, Psychiatry and other LTHC staff to assist you in connecting with outside services – including assisting in scheduling and attending appointments with outside providers, helping you meet your healthcare needs and providing Peer Counseling Services to help you meet your health and wellness goals.  Talk with your PCP to discuss a possible referral.

Community Health Workers

Our Early Relational Health, Community Health Worker (ERH CHW) works with children and families to provide culturally responsive developmental, strengths-based health promotion and care coordination for children and families from birth to age 5. They’re available to help families navigate the often complex medical system and helps to break down silos and barriers to care while empowering families to continue to advocate for themselves within these systems.

Our K-12 Community Health Worker – As the K-12 CHW works with youth and families to provide culturally responsive developmental, strengths-based health promotion and care coordination for school aged youth and their families. They’re available to help youth and their families navigate the often complex medical system and helps to break down silos and barriers to care while empowering youth and their families to continue to advocate for themselves within these systems.
